Inaugural Post

have been thinking about this for longer than I care to admit.

The honest version is this. Most of what I believe about AI and medicine does not fit neatly into a conference slide or an executive summary. It lives in the tension between what technology promises and what a real hospital unit will actually absorb. It lives in the conversations I have with colleagues who are brilliant and skeptical and overworked and still curious. Those conversations are where the real thinking happens, and for years they have stayed private.

That feels like a waste.

This is an attempt to fix that. Not a platform, or a brand. This is a working space closer to an open notebook than a polished publication, where I can put an idea down, let people push on it, and see what survives. I have spent the better part of a decade sitting at the intersection of clinical care and informatics implementation, which mostly means I have watched a lot of well-funded tools fail for reasons that had nothing to do with the technology. I have opinions about why and I want to share them.

What I am most interested in is collaboration. Not the conference panel kind, where everyone already agrees before the microphone is on. I mean the kind that happens when a mentor says something that reframes a problem you have been stuck on for months, or when a friend who works in a completely different field points out the obvious thing you missed. I want this to be a place where that can happen in writing, across time zones, without a meeting invite.

The topics will mostly orbit AI and healthcare, because that is where my work lives. But I do not expect it to stay tidy. The edges of that conversation bleed into medical education, clinical leadership, institutional change, and occasionally into what it means to practice medicine when the tools are changing faster than the culture. I will follow the thread where it goes.

If you are a clinician who is curious but skeptical, you belong here. If you are a technologist who wants to understand why implementation is harder than the demo, you belong here. If you are a researcher, a leader, a student, or someone who just cares about getting healthcare right, same.

I do not have all of this figured out. That is actually the point.
